Timeline for Transition

The transition to the Support at Home program will occur in phases to ensure a smooth changeover for all stakeholders:

From 1 July 2025 – The Support at Home program will replace the existing Home Care Packages (HCP) Program and the Short-Term Restorative Care (STRC) Programme.

From 1 July 2027 – The Commonwealth Home Support Programme (CHSP) will transition to the new Support at Home program. This staged approach allows HCP and STRC providers to prepare adequately for the transition and ensures CHSP providers have sufficient time to adjust their business systems and payment arrangements. This careful planning aims to minimise disruptions for clients and ensure a seamless transition to the new system.

Our nurses live in a lot of neighborhoods, including yours, and together we are there to care for you and your loved ones. Our nursing services include the following:

  • Comprehensive assessments
  • Post-hospitalisation care

  • Dementia care
  • Support for early diagnosis of depression
  • Chronic disease management (for example: asthma, arthritis, kidney disease, chronic pulmonary obstructive disease (COPD), coronary heart disease, dementia, diabetes, mental health conditions, osteoporosis, stroke).
  • Continence and catheter management
  • Rehabilitation
  • Health education
  • Palliative care
  • Grief and bereavement support
