Maintaining your ability to live at home as safely and independently as possible is our goal. Our team of support staff, care managers, nurses & allied health are here to assist you in this goal. Together we can work towards meeting goals by creating a plan together to achieve them. Reliable Home Care clients are at the centre of their care – our team of professionals work with our clients and their family to identify the services and supports most important to them to help meet their needs.

Our services may include:

  • Clinical Nursing - wound management, diabetes management, dementia care, medication management, nursing assessments, continence assessments, continence care, post hospital discharge care, clinical referrals to services
  • Allied Health – podiatry, occupational therapy, physiotherapy, exercise physiology, speech pathology, dietetics
  • Assistance with Daily Activities – personal care, transport, shopping, domestic assistance
  • NDIS Supports - Early Childhood Intervention with our NDIS partners
  • Supported Independent Living – via our NDIS partners
  • Personal Care – showering, grooming, continence care, personal care, meal preparation.
  • Domestic Assistance – cleaning, housekeeping, ironing.
  • Garden Maintenance – yard and garden maintenance. Once off rubbish removal. Minor weeding & minor hedging services.
  • Home Maintenance – Minor plumbing & electrical work, home modifications (subject to Occupational assessment)
  • Social Support – transport for shopping and medical appointments, getting to and from & support to stay with you
  • Community Participation & Outings – including social outings & transport to events/family functions & accessing the community
  • Mobility and aids – support & aids to keep you safe and mobile at home and out and about.
  • Consumables – continence products, clinical supplies, oxygen / enteral feeding supplies, specialised nutritional supplements.
  • Therapeutic Support – remedial massage & acupuncture services
  • Extras – The above may not list everything an individual needs. We are committed to tailoring a care plan to suit them within the home care funding framework. Our Clinical assessors will design a care plan to optimise the use of their home care package funding to deliver services & supports to our clients.
