About the Support at Home Program

The Support at Home program is set to launch in Australia on 1st July 2025 designed to consolidate several existing in-home care programs into a simpler, fairer system. The primary goal is to improve access to services, equipment, and home modifications, thereby helping older Australians remain healthy, active, and socially connected to their communities. The program was created in response to recommendations from the Royal Commission into Aged Care Quality and Safety and focuses on providing timely, equitable, and higher-quality care. The program will replace the current Home Care Packages & Short-Term Restorative Care programs, focusing on helping older individuals live independently at home for as long as possible.

Nursing Services

We have a team of friendly and experienced nurses that have been trained to provide you with special nursing care wherever you feel the most comfortable. Our nurses live in a lot of neighborhoods, including yours, and together we are there to care for you and your loved ones. Our nursing services include the following:

  • Comprehensive assessments
  • Post-hospitalisation care
  • Wound care
  • Medication management
  • Dementia care
  • Support for early diagnosis of depression
  • Chronic disease management (for example: asthma, arthritis, kidney disease, chronic pulmonary obstructive disease (COPD), coronary heart disease, dementia, diabetes, mental health conditions, osteoporosis, stroke).
  • Continence and catheter management
  • Rehabilitation
  • Health education
  • Palliative care
  • Grief and bereavement support
